Notice how the matter from the star does not fall directly into the black hole because the star has angular momentum from orbiting with the galaxy, also notice the spiral created by the expelled matter. It's caused by the differing orbital velocities of matter throughout the star due to the diameter of the star. That close to a black hole, even such a tiny difference as a few million km can have massive effects on the orbit. I'm an engineer in college and orbital mechanics is one of my subjects.
