The most basic mechanism we use to catch it is IP address. Every account has a list of associated IP Addresses, so accounts can be linked together this way.
There are more advanced methods, but most is caught that way, since most people doing multi-account voting aren't making any attempt to hide it, they don't realize we can see it.
